Dear {jury commissioner}:

I have been summoned to serve as a juror on {date} and must respectfully request a postponement.

I gave birth on {date}, and my newborn is breastfeeding exclusively and unable to take a bottle. Moreover, I am experiencing {medical issue} related to the birth that will take some time to resolve.

My physician anticipates that I will be able to sit on a jury in {time period}, and I have attached a note to that effect. I would greatly appreciate a postponement until {date} or later. I will be fully recovered and have child care and thus would be happy to serve at that time.
