IRA Hardship Withdrawal

Dear {investment entity or employer}:

I would like to take a hardship withdrawal from my IRA account, plan number {number}, in the amount of {amount} due to an immediate and heavy financial need.

I can readily provide documentation, but in short, I am {paying for a funeral/have high medical bills/am newly disabled/etc.}. Accessing my IRA is my last resort.
