To Whom It May Concern:

I am writing to try to {consolidate my loan, avoid foreclosure, restructure a payment plan, delay payment, request debt forgiveness} for {my mortgage, my personal loan, car loan, credit card bill, medical bill etc.}.

On {date}, a {fire/flood/earthquake/tornado} caused significant damage to my house. Property damage included {brief list and description of damages}. Please see the list of damages attached to this letter for a full description.

I {did/did not} have insurance for this eventuality and the total out-of-pocket expense is costing me {amount}. A statement from my insurance provider is attached and confirms my financial situation.

With this new, unexpected expense, I cannot hope to meet my monthly {mortgage/car/etc.} payments. I have worked very hard to make sure that I always have a fallback plan for paying my bills, but I hope you can understand that this was a massive, unforeseen setback that needs to be taken care of immediately. My family and I cannot live without our house.

{If this is for a mortgage, specify whether you'd like to stay in your home, or whether the home is up for sale already. For any other loan, come up with some sort of plan for repayment, such as lower payments each month, a lower interest rate, etc., and suggest it here.}
